Release of Information Form

Student information, including academic progress, accommodations, and any other information relating to their time at StFX is confidential. The university will not disclose any information about a student to a third party before we receive expressed written communication (i.e. a completed Release of Information Form) from the student allowing a representative from the university to speak to a third party about the student.
